<p>From a narrative standpoint, Itachi would still meet his demise like he did only this time it would be less a case of the tragic hero throwing the fight all in the name of protecting Sasuke and moreso Sasuke finally serving justice as Kishimoto would not kill off Sasuke. Sasuke probably wouldn't have reached that moral event horizon as Obito would have no big truth bomb to drop on him and I think Sasuke would probably make his way into becoming an Anti Hero or something less antagonistic as he did in Canon. Of course the series would definitely be notable altered with this alternative Sasuke and no Itachi pulling the hero in the War Arc but yeah from a storytelling standpoint Itachi was the obstacle for Sasuke to overcome. </p><p>I mean sure if you want to disregard the narrative yeah okay Itachi would kill Sasuke but he'd still be dying of that disease so really it's in Itachi's favour he wasn't written so one-dimensionally. </p>

<p>Considering a creepy snake guy that doesn't usually die wants possesion of his body, I think any sane guy would go for a kill, even the insane Itachi Uchiha. Though the thing is, Orochimaru is not just immortal, he's sneaky, like he's always hidden, so Itachi will never be able to find him out to kill him. Maybe that's why real nice guy Itachi couldn't kill him off either, plus even the Akatsuki couldn't find Orochimaru properly, it was only Jiraya who was able to, if I'm right. <p>Well Itachi was probably written stronger being a good guy and all, the plot of Sasuke and Itachi, needed a strong Itachi that wasnt using his full power against Sasuke. The evil Itachi would not need such a boost, and so, the fight between him and Sasuke would be his physical best/point. Basically, Sasuke would win, without Itachi backing down. In the end, there wouldn't be a MS Sasuke for what we know. And so, the plot would differ, with many of the exciting highlights of the show being missed. </p>

<p>@Jason </p><p>What would Itachi's goal then be? To be evil isn't really a goal. I'd guess whatever his goal were to be, he would accomplish it in any manner necessary. Probably even persuade Sasuke to join his cause. </p>

Considering he is bloodlustes he is more likely to use the MS and each use would be more intense. Considering he could barely see Sasuke in their fight he may go blind before that point and end up mostly harmless.
