Naruto been stronger than sasuke from the get go like that's why Sasuke left the hidden leaf village to become stronger because Naruto is stronger than him so of course Naruto would have won at the hospital.
Yeah. It's more like almost losing reinforced Sasuke's survivors guilt that he was weak and couldn't protect his clan and needed to do anything to extract revenge. People like to say it was out of jelousy
Technically, if Kakashi didn't step in, they would've killed Sakura when their techniques collided, so I'm pretty sure the fight wouldn't last any longer.
