That's just one of those background frames. There are a lot of moments where real adult Naruto looks actually badass. You shouldn't compare a quality frame to a sloppy one.

Adult naruto has never looked good. The main thing is the hair, if they gave him better hair and kept everything else it actually looks fine. But between shippuden and the last his face got so much skinnier for some reason. That didn’t happen to minato so why should it have happened to him? Boruto looks fine with a skinnier face, but if you compare 16 year old boruto to 12 year old boruto the difference is really noticeable and you can’t unsee it. I’ve probably gone on about this for way too long, but the main problem with naruto is his hair and why did his face get so much skinnier when minatos didnt.

Maybe the anime just deliberately makes him look like shit. He looks better in the manga.
