Kaguya would still lose because she’d 100% want madaras chakra reserves(kinda like how it played out originally) and I’m guessing madara would either turn out exactly like sasuke wondering the world to atone for his sins or imo very likely think back to where he mocked hashirama to kill himself or tobirama if he wanted peace between the clans and commit suicide as it is another way to atone for his sins and bring peace which was what he wanted in the first place
When it comes to the boruto series approaching 2 decades later he wouldnt be more of a threat to the villains than sasuke himself. Perhaps he would’ve gotten strong enough to take on kaguya by himself if he had lived but that is very head cannon-y but ig it would be cool to see madara and sasukes relationship in the boruto era
