The hokage is the one acknowledged by the village; it's someone who is the leader people want. It's based upon what deeds you achieve, not about what skills you possess, even though often you possess the right skills to achieve your deeds anyway.

Pierrot can add whatever they want: as long as it doesn't conflict with Kishimoto's ideas. E.g. they can't kill off a character that's supposed to live later or introduce a jutsu that may conflict with other stories (e.g. Naruto learning other releases). Other than that, pretty much everything is free game.
Secondly, Naruto's not a Jonin yet, and may never be.
